If you haven't noticed, at the top of all of the CMS 484 (commonly referred to as DMERC Style) Certificates of Medical Necessity (CMN's) they are set to expire 2/2020. While we are waiting on CMS to release new forms, national committees and MAC's are advising Providers to continue to use the current form. 

We will update everyone when new forms are released, so that you can get them into HDMS.
